Medicaid CDS

Consumer Directed Services (CDS) is Missouri Medicaid's program for self-directed home care. Many families use it to be paid for caring for a loved one.

Most Popular Use Case

Family member as paid caregiver.

Through CDS, an adult child, sibling, or other family member can be hired and paid as your caregiver. (Spouses are excluded by Missouri rule.)

How to Start

Getting set up on CDS.

Eligibility check

You must be on (or eligible for) MO HealthNet and able to direct your own care (or have a designated representative).

Caregiver onboarding

We help your chosen caregiver complete the application, background check, and required training.

Care begins, paychecks start

Once enrolled, the caregiver starts logging hours. We handle payroll, taxes, and reporting.

CDS basics.

Can my spouse be my paid caregiver?

No — Missouri Medicaid CDS excludes spouses from being paid CDS caregivers. Other adult family members (children, siblings, etc.) typically can.

How much will my caregiver be paid?

Rates are set by Missouri Medicaid and vary by service type. We'll explain the current rates during your consultation.

What if my caregiver wants to quit or change?

You can change your CDS caregiver. We help you onboard the next one without a gap in care.

Do I have to be the one signing time sheets?

You (or your designated representative) review and approve caregiver time sheets. It's how Missouri keeps the program honest.
